Should you desire to join The TV Connection for 6 months at a time, we'll give you one of those months free, so your start-up fee will only be $50.00. Once you have been online for six months and achieve seniority status, the 6-month renewal fee will be $25.00 if we bill you by U.S. Mail, or $20.00 if we automatically debit your credit card at the beginning of each six-month period, and you'll still receive one month free during each 6-month period.
If you want to become a member of BOTH systems (there are different members as well as an overlap on both), the second system's monthly fee is half that of the first system. In other words, if you choose to be billed by U.S. Mail, the charge would be $15.00 per month for the first six months, $7.50 thereafter. Members on automatic credit card payment would be charged $14.00 per month for the remaining five months of their initial period (the first month charge is still $15.00), $6.50 thereafter.
Rates for both systems using the 6-month period payment plan are $75.00 for the initial sign-up fee with one month free on both systems, $37.50 per 6-month period with one month free starting with month #7 if we bill you via U.S. Mail, or $32.50 per 6-month period starting with month #7 with one month free on both systems if we automatically charge your credit card semi-annually.
When filling out the form below, please specify whether you wish your initial sign-up period to be for 1 or 6 months.
If you wish to use your credit card and pay via fax, please fill out this form, print and fax it to us at (818)701-0560. If you wish to pay by check or money order, please fill out your name and address, specify whether you want to sign up for 1 or 6 months in the space provided, and in the "special instructions" box, please type "Request sign-up package by U.S. Mail." Then click on the "Send Form" button, and the start-up forms will be mailed to you.
1. By subscribing to The TV Connection, you become a
Member, and agree to be bound by this agreement. "TTVC" is an interactive
information and transaction service accessible by touch-tone phones (voice mail system) and via computer and modem (Web BBS). Shokus Video is the provider of TTVC.
2. Members are responsible for payment of periodic service
fees according to the then-current billing terms. Shokus Video reserves the
right to change fees at any time, effective upon 30 days notice to the Member.
Members are responsible for payment of charges for the connection time
charged by either the Member's local phone company or long distance carrier.
3. Delivery of TTVC may be terminated at any time, and
without cause, by either Shokus Video or a Member upon notification to the
other. Members are liable for any charges incurred by them until termination
of service. Terminations by the Member must be made in writing either
by statement or by returning an invoice with the word "CANCEL" written on it
within 10 days after receipt. Membership shall be terminated if payment is
not received within 12 business days upon receipt of invoice by Member.
All payments are made in advance.
4. Access to and use of TTVC is through a combination of a
Mailbox Number and Password. Shokus Video will assign both to the Member.
The Mailbox Number is your "address" on TTVC and will be disclosed to others.
Each Member must keep his issued Password strictly confidential for security
reasons. The Password will allow access to the Member's personal messages (voice mail system), and to posts on the public boards (Web BBS)
so REMEMBER YOUR PASSWORD! TTVC will assign a new Password if forgotten or
discovered by another Member. Members abusing Password privileges are
subject to termination.
5. TTVC enables Members to share information with other
Members. Members agree not to record or post any defamatory, inaccurate, abusive,
obscene, profane, sexually explicit, threatening, or racially offensive
material. Transmission of material that violates any federal, state, or
local law is prohibited, is a breach of this Agreement, and will result in
immediate termination of membership. TTVC reserves the right to review and
edit any recording (voice mail) or article (Web BBS) posted on the public bulletin boards. TTVC reserves the right, without liability, to remove any material on the public boards at the
sole discretion of TTVC. Further, no personal data regarding any Member
including but not limited to private telephone numbers or addresses are
permitted on the public boards on the voice mail system at any time. Private information given out in articles on the Web BBS are at the Member's discretion only, and TTVC shall not be held responsible for any misuse or unbecoming conduct as a result. Further, TTVC shall have no liability for
information and/or transactions shared between Members through their public and private
messaging, and each Member agrees to hold TTVC and Shokus Video harmless and
indemnify both from any claim or action arising from such transactions.
TTVC will not intentionally inspect the contents of private messaging sent
by one Member to another (voice mail system only; Web BBS not applicable) or disclose such contents to other than the sender or an intended recipient, without the consent of both parties, unless
required to do so by law. Private messages are defined as messages sent by
one Member to other Members by means of personal Voice or e-Mailboxes. TTVC
reserves the right to delete both private (voice mail system only) and public messages (both systems) after a period of time established by Shokus Video/The TV Connection.
6. Notices by TTVC to Members may be given by means of
voice mail to private mailboxes (voice mail system), by general postings on the public boards of each system, by e-mail, or by U.S. Mail. Notices by Members to TTVC may be given by U.S.
Mail, by e-mail, or directly to the Shokus Video/TTVC Voice Mailbox. Voice
mail messages should be addressed to Box #000 on the voice mail system; e-mail
addressed to ttvc@shokus.com
, or U.S. Mail sent to: Shokus Video/TTVC, P.O. Box 3125, Chatsworth, CA
91313.
7. Any liability of TTVC, including without limitation any
liability for damages caused or allegedly caused by any failure of
performance, error, ommission, interruption, deletion, defect, delay in
operation or transmission, communications line failure, theft or destruction
or unauthorized access to, alteration, or use of records, whether for breach
of agreement, tortious behavior, negligence, or under any other cause of
action, shall be strictly limited to a pro-ration of the amount paid on
behalf of the Member to TTVC for the usage of TTVC during any billing
period. TTVC is not responsible for rebates resulting in the downtime
caused by equipment malfunctions unless the period exceeds 72 hours and is
not an act of God. No warranty or liablity is made by Shokus Video/The TV
Connection regarding any materials received by a Member from other Members,
including, but not limited to copyrights by others (if any) and conditions
of materials as a result of private or public messaging on either or both systems between
members.
